- The Lancet Countdown’s sixth annual report tracks 44 indicators of health impacts that are directly linked to climate change – and shows key trends are getting worse and exacerbating already existing health and social inequities.

The 2021 report of the Lancet Countdown on health and climate change: code red for a healthy future outlines the growing risks to health and climate. These risks exacerbate the health hazards already faced by many, particularly in communities exposed to food and water insecurity, heatwaves, and the spread of infectious diseases. The authors call for urgent, globally coordinated action to mitigate climate change and build a healthier, sustainable future for all.
- Many current COVID-19 recovery plans are not compatible with the Paris Agreement and will therefore have long-term health implications.
- Despite the detrimental climate effects, the world continues to subsidise fossil fuels. In 2018, 65 out of the 84 countries analysed by Lancet Countdown researchers had net-negative carbon prices equivalent to an overall subsidising of fossil fuels. The median value of the subsidy was US$1 billion, with some countries providing net subsidies to fossil fuels in the tens of billions of dollars each year. The 84 countries surveyed are responsible for around 92% of global CO2 emissions.
- In 2020, adults over 65 were affected by 3.1 billion more days of heatwave exposure than in the 1986–2005 baseline average. Chinese, Indian, American, Japanese, and Indonesian senior citizens were the most affected.
- Climate change and its drivers are creating ideal conditions for infectious disease transmission, potentially undoing decades of progress to control diseases such as dengue fever, chikungunya, Zika, malaria, and cholera.
- Healthcare systems are ill-prepared for current and future climate-induced health shocks. Only 45 (49%) of 91 countries in 2021 reported having carried out a climate change and health vulnerability and adaptation assessment.
The COVID-19 pandemic has demonstrated the need for increased international co-operation in the face of global crises. Politicians must show leadership by moving beyond rhetoric and take action at the upcoming UN Climate Change Conference (COP26), which will start on Sunday 31 October 2021 in Glasgow, Scotland. Carbon emissions must be rapidly reduced to improve health and to provide a more equitable, sustainable future.
As countries commit trillions of dollars to restart their economies in the midst of the COVID-19 pandemic, the report urges political leaders and policy makers to use this public spending to reduce inequities. Promoting a green recovery by creating new and green jobs, and safeguarding health, will build healthier populations now and in the future.
A fossil-fuel driven recovery – which includes large subsidies for oil, gas and coal and limited financial support for clean energy – could potentially meet narrow and near-term economic targets, but may then push the world irrevocably off course and make it impossible to meet the maximum 1.5C of warming as outlined in the Paris Agreement. This has a toll on human health, hardest hitting to those people living in low income countries, whose populations have made the smallest relative contribution to climate change. As governments turn from emergency spending to long term post-pandemic recovery it is vital that more of these funds are spent in ways that reduce climate change, such as promoting jobs in zero-carbon energy, where investment lags behind what is necessary to keep within 1.5C of warming.
The Lancet Countdown report shows that many countries are under-prepared for the health effects of climate change. In a 2021 World Health Organisation survey of health and climate change, only 45 of 91 countries surveyed (49%) say they have a national health and climate change plan or strategy. Only 8 out of those 45 countries in the analysis reported that their assessments of the effects of climate change on their citizens’ health had influenced the allocation of human and financial resources. The survey found 69% of countries in this analysis reported insufficient financing was a barrier to implementing these plans.
“Climate change is here and we’re already seeing it damaging human health across the world,” said Prof Anthony Costello, Executive Director of the Lancet Countdown.
“As the COVID-19 crisis continues, every country is facing some aspect of the climate crisis too. The 2021 report shows that populations of 134 countries have experienced an increase in exposure to wildfires. Millions of farmers and construction workers could have lost income because on some days it’s just too hot for them to work. Drought is more widespread than ever before. The Lancet Countdown’s report has over 40 indicators and far too many of them are flashing red.
“But the good news is that the huge efforts countries are making to kick-start their economies after the pandemic can be orientated towards responding to climate change and COVID-19 simultaneously. We have a choice. The recovery from COVID-19 can be a green recovery that puts us on the path of improving human health and reducing inequities, or it can be a business-as-usual recovery that puts us all at risk.” 
The Lancet Countdown report represents the consensus of leading researchers from 38 academic institutions and UN agencies. The 44 indicators in the 2021 report expose an unabated rise in the health impacts of climate change:
- The potential for outbreaks of dengue, chikungunya and Zika is increasing most rapidly in countries with a very high human development index, including European countries. Suitability for malaria infections is increasing in cooler highland areas of countries with a low human development index. Coasts around northern Europe and the US are becoming more conducive to bacteria which produce gastroenteritis, severe wound infections, and sepsis. In resource-limited countries the same dynamic is putting decades of progress towards controlling or eliminating these diseases at risk.
- There are 569.6 million people living less than five metres above current sea levels, who could face rising risks of increased flooding, more intense storms, and soil and water salinification. Many of these people could be forced to permanently leave these areas and migrate further inland.
Maria Romanello, lead author of the Lancet Countdown report, said:
“This is our sixth report tracking progress on health and climate change and unfortunately we are still not seeing the accelerated change we need. At best the trends in emissions, renewable energy and tackling pollution have improved only very slightly. This year we saw people suffering intense heatwaves, deadly floods and wildfires. These are grim warnings that for every day that we delay our response to climate change, the situation gets more critical.
“Governments are spending trillions of dollars on the recovery from the COVID-19 pandemic. This gives us an opportunity to take a safer, healthier, low carbon path, but we have yet to do so. Less than one dollar in five being spent on the COVID-19 recovery is expected to reduce greenhouse gas emissions and the overall impact is likely to be negative. We are recovering from a health crisis in a way that’s putting our health at risk.
“It’s time to realise that no one is safe from the effects of climate change. As we recover from COVID-19 we still have the time to take a different path and create a healthier future for us all.” 
A Lancet Editorial adds, “The world is watching COP26—widely perceived as the last and best opportunity to reset the path to global net zero carbon emissions by 2050—and public interest in climate change is higher than ever, in part due to global youth activism and engagement…This year’s indicators give a bleak outlook: global inequities are increasing, and the direction of travel is worsening all health outcomes. Health services in low-income and middle-income countries are in particularly urgent need of strengthening…However, the future is not necessarily hopeless…Succumbing to the climate emergency is not inevitable.”
Key report findings
Just as the world is failing to deliver an equitable supply of COVID-19 vaccines, the data in this report exposes similar inequities in the global response to climate change. In general, it is the countries lowest on the human development index that are often least responsible for rising greenhouse gas emissions and are lagging behind in climate change adaptation and mitigation efforts and in realising the associated health benefits of accelerated decarbonisation.
- In 2020, up to of 19% of the global land surface was affected by extreme drought in any given month, a value that had not exceeded 13% between 1950 and 1999.
- Climate change is driving an increase in the frequency, intensity, and duration of drought events, threatening water security, sanitation, and food productivity, and increasing the risk of wildfires and exposure to pollutants. The five years with the most areas affected by extreme drought have all occurred since 2015. The Horn of Africa, a region impacted by recurrent extreme droughts and food insecurity, was one of the most affected areas in 2020.
- Climate change threatens to accelerate food insecurity, which affected 2 billion people in 2019. Rising temperatures shorten the time in which plants reach maturity, meaning smaller yields and an increased strain on our food systems. Maize has seen a 6% decrease in crop yield potential, wheat a 3% decrease and rice a 1.8% decrease, compared to 1981 – 2010 levels.
- Average sea surface temperature has increased in the territorial waters of nearly 70% (95 out of 136) of coastal countries analysed, compared to 2003-2005. This reflects an increasing threat to their marine food security. Worldwide 3.3 billion people depend on marine food.
- In 2021 the World Health Organisation found just over half of countries that answered to the Health and Climate Change Global Survey (37 out of 70) had a national health and climate change strategy in place, a similar proportion to 2018. Nearly three-quarters of countries surveyed said finances prevented them developing such a strategy, with others citing a lack of skilled people, being restricted by COVID-19 and lacking research and evidence.
- Globally, climate change adaptation funding directed at health systems represents just 0.3% of total climate change adaptation funding.

The Lancet is a boil on the medical profession that needs be lanced.
It’s not just The Lancet. The entire medical profession has exposed itself during this Covid-19 episode.
If there is one thing that I have learned these last 18 months it is that the medical profession (or “sickness industry” as I now prefer) is largely corrupt. Doctors (at least in the US) are trained by medical schools funded by the pharmaceutical industry and, as a result, are largely pill pushers. Non-profits are all about the money. Insurance companies are partners in maximizing expenditures. And leftist/statist politics drives decisions at all levels.
It would be wise to focus on educating oneself about health matters. Get your diet right, exercise appropriately, and listen to your body.
All medical and scientific journals need to become transparent and identify with every article who peer reviewed it. This would help the public to identify the poorest reviewers and be able to expose and discredit them and the editors of these journals who allow this.

Don’t forget that fresh from her 10 year stint as Sec.Gen, of the UN Framework Convention for Climate Change, Christiana Figueres joined the Lancet in 2017 as Chair of its High Level Advisory Board, where her role is “to help guide the Lancet Countdown to maximize its impact and deliver on the promise of the Paris Agreement”.
Despite never being elected, she has had immense influence over people’s lives, as she has many advisory roles worldwide. At a 2017 news conference in Brussels, she admitted that the goal of environmental activists is not to save the world from ecological calamity but to destroy capitalism. She said:”This is probably the most difficult task we have ever given ourselves, which is to intentionally transform the economic development model for the first time in human history.”
These people are ubiquitous, moving from one influential position to another, almost interchangeably, whilst few notice.

Ten years ago I commented elsewhere about the pack of lies Christiana Figueres was telling when she was involved at COP 17 in Durban. She is either woefully ignorant or deliberately deceitful or perhaps both. It is utterly shameful that there are no good journalists pointing these out to her and publishing her responses or refusals to answer in the mainstream media.
100% correct. The ‘potential for … outbreaks is increasing’ along with the usual AGW misinformation is a clear indicator that this is complete speculation built on a badly flawed premise.
One thing that demonstrated that this study was off in green fantasyland was denouncing “subsidies” for fossil fuels. They are a favorite green unicorn, something that everyone has heard of but exists only in the imagination of the greens.
In the minds of the left, the fact that they haven’t been taxed out of existence is proof that they are being subsidized.
The oil and gas industry is the most taxed worldwide. In USA and other tax and royalty regimes, the country and state get 12-20% tax of gross revenue. If mineral interest is on federal or state land, government gets additional 12-30% of gross revenue. Then after products are refined, there are exorbitant taxes on products as gasoline. If oil and gas companies make profit after all this gross taxing, operating costs, and investment, they pay additional income tax.
The few legal deductions allowed for income tax are called “subsidies” by the greenies, in addition to theoretical damages for climate damage. This is a farce.
Subsidies conversation is a smoke screen. Who will pay for the roads, bridges, education… if or when the oil and gas industry goes away. Certainly not the windmill, solar panel, biofuel (which are heavily subsidized) or even coal, hydro and nuclear industries.
Follow the money. When oil and gas industry disappears, world will be in worse place with this over taxed cash cow gone.
I just saw a report about California gasoline prices and they showed one pump was charging $8.50 for a gallon of premium gasoline.
Most places are paying $3 to $5 per gallon. So it looks like California is paying a lot more in gasoline taxes than other States, although that $8.50 price might be an outlier for all I know.
And Biden said last night he doesn’t see any way to get the gasoline prices down. Hey Joe, give Trump a call. He can tell you how to do it. I was paying less than $2 per gallon when Trump was in office. Now, the price is about $3.25 per gallon.
In France train line are often made slightly below average ground level and the noise goes upward; sometimes high speed lines have to be built higher and the noise travels far away. This is a big nuisance as high speed trains are very noisy, on the full length of their travel (OTOH you can only hear airplanes when they are near the ground).
Is that nuisance a rail subsidy? Nobody to my knowledge has ever described it as one.

The claims of fossil fuel subsidies fall into two classes. One is deliberately not understanding cost and tax accounting, as fossil fuel companies are not treated any differently than other companies.
The second is to make up a “social cost of carbon” , and then claim that the companies are being subsidized by not paying said nonexistent tax.
A third one is counting as subsidies, the gift of free gasoline, and energy by a few countries in Mid-east, possibly Venezuela. They do this since they are awash in fossil fuels and also to help keep themselves in power. But, it is a totally different kind of “subsidy” than for solar, wind, etc. It is a subsidy to the citizens of those countries, not to the companies.

When I have asked a greenie about what subsidies we in the oil induistry receive, and to give some examples they quote for the north sea-
1) Offsetting oil extraction expenditures against taxable revenue
2) Writing off depreciation of finite lefetime infrastructure against revenue
3) Tax hedging a sum set aside for abandonment costs
As far as I can see these are quite normal business expenses such as say a manufacturing plant -if we have any left would factor into its tax return.

This a very valuable metric, even though it is incorrectly phrased. It should say “…3.1 billion person-days of heatwave…”
So what is extremely relevant is the same metric for chronic underheating and extreme cold. What we will find looking at that metric is appalling in terms of the direction and magnitude for developed countries. Winter season excess deaths in the UK are up about 50% in the past 5 years due to the rising cost of energy. This winter, expect the situation to be even worse.
Something like 11% of all deaths in China are attributable to chronic underheating. Chronic underheating, defined by the WHO as a room temperature under 18C, is widespread in Asia, and of growing concern in the rich northern countries experiencing a rapid rise in the cost of energy, due primarily to government policies that deliberately do that.
Occasional articles posted at realclimatescience.com detail the appalling death rate during heat waves 100 or more years ago. Technology has largely eliminated this risk. When it comes to heat, adaptation is far cheaper than ineffective attempts at mitigation.
Now that we are entering a decades-long climate cooling phase we will see that adaptation is much more difficult than for warming. Check the price of food. Even a politically charged publication such as the Lancet will have to cover the drop in nutrition caused by crop failures due to early frosts and late springs. The damage in South America this year is amazing. Installing a heat pump will not grow food.

The only thing helping the food supply during cooling is the relative abundance of CO2 compared with the last major turn-down 210 years ago.

It’s worth reading Norman Borlaug’s biography, if only for the first chapter that discusses the proportion of American’s directly involved in agricultural production on farm. Roughly 8/10 jobs were on farm. Currently, in Canada (and assuming same for U.S.) only 1 in 8 works in an agriculturally related field, the majority in urban centers working in regulation, sales, marketing etc. but not in actual production. That’s what Greta’s ilk is asking for without recognizing it.

I read a little while back that the world actually produces enough food to feed the world’s population amply, the issue appears to be one of distribution, not food production, thanks to modern farming methods & practices, methods & practices the Globalists & their Useful Idiots want to see fail, in a Malthusian effort to cause mass murder, err, sorry, population reduction through food restrictions. ripshin – I suspect Dynamo is referring to the publication of the fraudulent HCQ “study” last year that they had to retract.
Either that or the fraudulent 1998 article “MMR vaccine causes autism” that they had to retract. The Lancet has a history of publishing such fraudulent studies.
I understood that the apparent rise in cases of autism was mostly if not entirely a result of better identification/diagnosis of the condition, & increasing the breadth of definition of it!!! How many other syndromes or conditions have been increased as a result in changes in defining said syndrome or condition???
I am baffled that so many children at school with me sixty years ago managed to “hide” their autism and ADD and cope in schools where there was no mollycoddling of children. Perhaps these only appeared from the eighties? Similarly many problems that are today linked to mental health were previously quite a normal part of life for most of us and did not need specialized treatments.

As a physician I am dismayed that a once respected and responsible preeminent health sciences journal has abandoned science in favour of propaganda that aims to destroy modern society based on the myth of a climate emergency driven by human CO2 emissions. Where is the evidence? All of their claims about human health impacts are just predictions and indirect associations. Nearly every measurable aspect of human wellbeing (possibly excluding common sense) has been continuously improving while the global surface and low atmosphere has gently warmed a small amount. They talk about heat waves and ignore that cold is more deadly. They talk about decline in crop yield potential rather than reporting on the well documented increase in all crop yields with attendant decline in hunger. They are fabricating a nonexistent emergency. If we follow their lead we will destroy modern society and lose all the gains for both humans the environment we have achieved over many long decades of effort. Shame.

Poverty is caused by totalitarian regimes and making the west poor will only lead to poor people becoming poorer as aid from the west dries up. Also, COVID vax would not exist if the west was made poor and destitute. Whilst this would equal equity, it is the worse case outcome for humanity and yet this is what these Marxist fools are seeking.

I remember in the 1970s when three of us got a paper published in Lancet. It was about bone grafting. I did the histology and the other two authors did the surgery.
